THIS FORM MUST BE COMPLETED IN ORDER TO REGISTER YOUR ENGINE FOR ANY FUTURE WARRANTY CLAIMS. ALL INFORMATION MUST BE COMPLETE OR IT WILL NOT BE PROCESSED.

NO WARRANTY CLAIMS CAN BE STARTED WITHOUT THIS INFORMATION

DEX warranty for renovated engines includes only components inspected and/or replaced by DEX during the renovation process.

The following items are covered:

  • Rod and main bearings
  • Lower idler gear
  • Engine wiring harness
  • Crankshaft
  • Injectors
  • Pistons, Piston Rings and Liners
  • Injector Cups
  • EGR Cooler
  • Double idler gear

Warranty does NOT cover any part not listed, mating parts, seals and gaskets, fuel, oil, coolant, contamination, failures or leaks, and associated parts like alternators, starters, turbochargers, electrical ECU/sensors, and other mounting/mounted parts/components.

PRIOR AUTHORIZATION BY DEX IS REQUIRED BEFORE PERFORMING ANY DIAGNOSIS OR REPAIRS. PLEASE CONTACT DEX CUSTOMER SERVICE FOR PRIOR AUTHORIZATION PRIOR TO ATTEMPTING ANY DIAGNOSIS OR REPAIR.

  1. Limited Warranty is not valid until this form AND a copy of the INSTALLATION REPAIR ORDER OR INVOICE is received by DEX Heavy Duty Parts. Warranty begins at the date of installation or 30 days from date of shipment, whichever comes first.
  2. Please e-mail completed form to: customerservice@dex.dev.velaagency.com or Fax to: 336-291-9011 along with a copy of the installation repair order or invoice***
  3. Installation RO or installer invoice # (copy must be attached to this form to finalize registration process)
  4. The information used in this form will be used to determine warranty coverage and payment should a claim occur. Incomplete information will not be processed.
